Liability Release Form

For Participation in Street Ministry Activities This Liability Release Form ("Agreement") is made and entered into by the participant ("Participant") in favor of Life Center, its directors, officers, employees, volunteers, agents, and affiliates (collectively, "the Ministry"). 1. Assumption of Risk I understand that participating in street ministry activities may involve risks, including but not limited to: Interaction with unknown, individuals, Unpredictable public environments, Potential verbal or physical confrontations, Exposure to illness, inclement weather, or unsafe conditions. I voluntarily assume all risks associated with my participation in these activities. 2. Release of Liability I hereby release, waive, and discharge the Life Center, its leaders, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action related to injury, illness, death, or property damage that may occur as a result of my participation. 3. Medical Treatment I authorize the Ministry to seek medical treatment on my behalf in the event of an emergency. I understand that I am responsible for any medical costs incurred. 4. Conduct & Compliance I agree to follow all instructions given by Ministry leaders and to conduct myself respectfully and lawfully while participating in street ministry activities. 5. Indemnification I agree to indemnify and hold harmless the Ministry from any claims, damages, or losses resulting from my actions during participation. 6. Photo & Video Release I grant permission for the Ministry to use photographs or video recordings of me for promotional or documentary purposes.
